Cardiovascular Disease includes angina, heart attack, Transient Ischaemic Attack (TIA), Cerebrovascular Disease (CVD – stroke), Peripheral Vascular Disease (PVD – claudication) and a few other conditions affecting the heart and circulation.
Patients with these diseases are on a register and are seen twice year – once near their birthday and then again six months later by the Practice Nurse or Healthcare Assistance (HCA).
Patients on the register will received a call-up notification to make an appointment with the Practice Nurse or HCA for their CVD review. The review includes checking your recent blood results for liver function, kidney function, and blood fat levels including cholesterol.
So that the results are available to be discussed at the appointment you may be asked to attend for a blood test with our Phlebotomist a week before.
